How to fill in the application form
The table below summarizes each field in the application form — including how it will impact your account.

Legal entity | Legal business name | The legal entity name as presented on your company’s registration document. |
Legal entity | Trading name | The name your customers know your company as. |
Legal entity | Type of company | Select the type from the drop down menu. |
Legal entity | Registration number | The registration number as presented in your company’s registration document. |
Legal entity | VAT number | The number which validates your VAT/Tax registration. |
Legal entity | Registration document | A government issued document proving the existence and registration of your business. |
Stores | Country | The country your store operates in. |
Stores | Address | Your store’s address. |
Stores | Postal code | Your store’s postal code. |
Stores | City | The city your store operates in. |
Stores | Phone | Your store’s phone number. |
Payout account | IBAN | The bank account that’ll receive your payouts. It needs to be based in the same country as your legal entity. |
Payout account | Proof of bank account | This document will be used by Adyen to verify your payout account.
It can be a voided cheque, a bank letter, bank statement, or a screenshot of an online banking environment. |
Ownership & control | Owners | Any natural person who ultimately owns more than 25% of your company’s shares. |
Ownership & control | Controlling persons | A natural person who has decision-making powers and is effectively in control of your company. |
Ownership & control | Signatory | A natural person authorized to sign documents on behalf of the company. They will sign the contracts and agreements issued by Adyen. |
